What we offer

We help people with incurable illness, supporting them and their families, friends and carers. We also support children and adults who are facing bereavement no matter what the cause of their bereavement.

SinglePoint is a flexible, responsive service for patients at end of life, that fits comfortably with other community services and existing hospice services delivering palliative care. SinglePoint is a St Helena community support service for patients with an incurable life limiting illness making it likely that they are in the last few months of life or are at risk of a sudden deterioration.

The service has two main functions:

1. Telephone advice line, 24 hours a day, 7 days a week
2. Rapid response service, 24 hours a day, 7 days a week

Aims of the service:

  • To process all referrals in to St Helena services
  • To support patients in the last few months of life through the provision of telephone advice, which may include signposting to a more relevant service
  • Admission avoidance when the usual place of residence is the preferred place of care and where acute hospital admission would fail to achieve an improvement or reversal of their general condition
  • To provide rapid response visits to those patients who are experiencing difficulties managing symptoms, a crisis in care or who are overwhelmed with psychological distress relating to their deteriorating condition, and who may be at the end of life. The service will also provide support for patients’ families and support networks and help find solutions to social crises
  • To provide a visit to patients referred to St Helena who are unable to wait for a planned response by another service and where delay would add additional distress, and where hospice intervention would provide access to specialist hospice services
  • To provide intensive support to those experiencing an on-going crisis situation or who may be waiting for a bed in The Hospice
  • To introduce advance care planning, including anticipatory prescribing when a quick deterioration precludes the GP putting in place in a timely manner and where omission would precipitate a crisis
  • To co-ordinate a response to a patient in a timely manner, when SinglePoint may not be the appropriate service to respond
  • Follow-up of patients when an RN has previously visited to ensure continuity of care and support
  • To prevent a crisis prior to initial assessment of a specialist hospice service
  • To host and maintain the My Care Choices Register
  • To manage the SinglePoint Virtual Ward

SinglePoint is a 24/7 advice and support helpline which helps to coordinate an individual’s care with the hospice. SinglePoint also works alongside other healthcare services such as GPs, Community Nurses or Specialists.
