Anyone over the age of 16 with a progressive life limiting, incurable illness can be referred to St Helena.
St Helena helps to support individuals, their family and carers affected by a progressive life limiting illness. Our primary goal is to optimise quality of life for all by the provision of physical, practical, emotional and spiritual support.
If you are a health or social care professional please complete an electronic referral including up to date clinical information, clinic letters and any recent hospital discharge summaries.

If you are asked by an individual or family member on how they can refer, please forward them to the self-referral form here or advise them to phone SinglePoint on 01206 890360 and ask to speak to a member of our Referrals Team. Please make them aware that if they are a family member they must obtain the consent of the individual they are referring.
Once the referral is received and all relevant information is gathered, the referrals team will then contact the individual. A decision will then be made as to what support from us is required, including signposting to other local services where appropriate.
